Is C.O.A required if an International student wants to marry a british citizen?

Is Certificate Of Approval really necessary for someone living in UK with a student visa to marry a british citizen? Can't they just get married and apply for the spousal visa? And what is the process and fees for the spousal visa?!

Sorry for too many questions, but I am confused about these things!
